You may use this form for any of the workshops listed at Lotus.  Make sure you write the name of the Instructor on top of the form. If you are having trouble seeing the form above call us at the number below.

If you are registering by Snail Mail and sending Check or Money Order, please email us to confirm receipt at lotusdancestudio@gmail.com. 

We do not recommend that you write your credit card information on form and send via regular US mail.  Please call the studio at 954-433-3552.  Please note that registrations taken over the phone where the actual credit card is not present will incur a charge of $1.50 per each registration.
